Access Healthcare is seeking a travel Physical Therapist for a travel job in Manitowoc, Wisconsin.
Job Description Requirements

Job Summary:
Evaluates and treats all referred Physical Therapy patients according to physician orders. Home Care: Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients.
Job Responsibilities:
-Consistently performs initial patient evaluations including administering appropriate tests/measures, identifies impairments and functional limitations. Establishes an initial plan of treatment in conjunction with the patient/caregiver.
-Actively appropriately selects and administers treatment strategies based on current physician orders and established treatment plan(s). Delegates appropriate tasks and supervises the PTA or HHA.
-Proactively consistently identifies functional improvement and goal attainment on a regular basis, supported by documented subjective and objective changes. Updates treatment plans and goals per department standards.
-Consistently completes documentation following department standards and timeframes. Documentation reflects skilled intervention and supports therapeutic interventions as reasonable and necessary.
-Is motivated to achieves productivity per department standards and completes job duties in an efficient and effective manner. Consistently takes full initiative to use non-productive time to benefit the entire department. Home Health Care: Consistently complies with applicable rules and regulations. Demonstrates accuracy with OASIS, adheres to the National Patient Safety Standards. Demonstrates responsible utilization of resources for each patient and actively participates in department PI activities.
Access Healthcare Job ID #3-36000928. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
