The clinics this RN supports include Janesville, Delavan, Edgerton, Evansville, Fort Atkinson, Stoughton, and Whitewater. Specialties include allergy, cardiology, dermatology, endocrinology, gastroenterology, infectious disease, pulmonary, sleep medicine, and wound. </p><p><b>Patient Population or Ratio: </b>The RN helps support between 4-15 providers at each location.</p><p><b>Schedule:</b> The schedule is variable due to the float role and varies from 7 a.m. to 6 p.m., typically with 8-hour shifts. Float RNs travel between departments clinics based on clinic needs.</p><p><b>Locations:</b></p><p>Delavan - 540 Bowers Blvd, Delavan, WI 53115</p><p>Edgerton - 11051 N Sherman Rd, Edgerton, WI 53534</p><p>Evansville - 10 N Water St, Evansville, WI 53536</p><p>Janesville - 3200 E Racine St, Janesville, WI 53546</p><p>Fort Atkinson - 740 Reena Ave, Fort Atkinson, WI 53538</p><p>Stoughton - 225 Church St., <i>Stoughton</i>, WI 53589</p><p>Whitewater - 1305 W Main St, Whitewater, WI 53190</p><p><b>Additional Job Duties: </b>This RN's current roles include triaging incoming/outgoing calls, assisting in InBasket work (including MyChart messages, results, and voicemail line), assessing and/or rooming patients, coordinating with core team members of the healthcare team, and providing support for the physician team.</p><p><b>Job Summary:</b></p>Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.<p><b>Job Responsibilities and Requirements:</b></p><p>PRIMARY RESPONSIBILITIES</p><ul><li>Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.</li><li>Serves as a primary coordinator of all disciplines for well-coordinated patient care.</li><li>Notes and carries out physician and nursing orders.</li><li>Assesses and coordinates patient's discharge planning needs with members of the healthcare team.</li><li>May round with physician in an inpatient setting.</li><li>Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient's age-specific needs and clinical needs as described in the department's Scope of Service.</li><li>As an SSM Health nurse, I will demonstrate the professional nursing standards defined in the professional practice model.
