Overview:

Were looking for a Registered Nurse, Admissions (Hospice) to join our team.

You will report directly to the Admissions Manager. You will be responsible for explaining the Companys services to patients and families in a manner that results in the admission of appropriate patients.

You will as an RN utilize the nursing process to contribute to each patients total plan of care.
You will visit patients and families in their place of residence or in the hospital to assess for appropriateness, explain services, monitor condition, or admit to hospice services in accordance with the admission procedure.
At least 2 years direct hospice experience as RNCM
Experience in completing admissions
Visit patients and families who are thinking about hospice and have difficult conversations. Explaining Gentiva/Heartlands philosophy and services.
Able to evaluate a patient and see hospice indicators for hospice eligibility, while painting picture to the medical director to establish eligibility
Fast paced between SOC, educating
Understanding the difference of palliative care and hospice and when is an appropriate time to transition
Educating Illumia NPs, RN and staff about hospice, they have no experience with hospice and the admissions nurse must have a strong knowledge with conditions of participation, key indicators and changing mindset of curative treatment to palliative and comfort

About You:

You have a Registered Nurse license in the state where the program is located.
Bachelors Degree and C.H.P.N. certification preferred.
Minimum of 1 year general nursing experience, Hospice experience preferred.
You have a minimum of 2 years Hospice Health Care or Medical Office experience, preferred.
CPR Certification

Compensation may vary within the salary range provided based on several factors including but not limited to a candidates location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range - $85,000 to $90,000
