Were seeking a Physical Therapist (PT) to provide skilled home health visits, assessments, and admissions to adult clients throughout Sussex County, including Georgetown, Millsboro, and Milford. Whether youre looking for full-time or part-time, we have a schedule to fit your life.

What Youll Do:
-
Visit clients in their homes to assess mobility, balance, and safety
-
Develop and implement individualized care plans and evidence-based interventions
-
Provide education to clients and families to support independence

Collaborate with a multidisciplinary care team

Document care electronically using a state-of-the-art tablet
What Youll Need:
-
Active Delaware PT license
-
Graduation from an accredited PT program
-
Ability to work independently, with strong communication and time management skills
