The PT will work closely with interdisciplinary teams to create and implement individualized treatment plans that align with residents goals and medical needs.Key Responsibilities:Conduct comprehensive assessments to evaluate residents physical conditions and functional limitations.Develop and implement customized treatment plans focused on improving strength, balance, coordination, and mobility.Provide hands-on therapy, exercise programs, and pain management techniques to enhance physical function.Educate residents, families, and caregivers on therapy techniques, home exercises, and injury prevention strategies.Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure integrated care.Maintain accurate documentation of evaluations, progress notes, and treatment plans in compliance with regulatory standards.Ensure a safe and supportive therapy environment, adapting interventions based on residents individual needs.Qualifications:Education: Doctorate or Masters degree in Physical Therapy from an accredited program.Licensure: Current state licensure as a Physical Therapist.Experience: Prior experience in geriatrics, skilled nursing, or a CCRC setting is preferred but not required.Skills: Strong interpersonal skills, teamwork, and the ability to motivate residents toward achieving their therapy goals.Certifications: CPR certification .
