Job Title: RN - Case Manager
Location: Jackson, MI
Duration: 13 weeks
Schedule: Days, 8a-4:30p, 40 hrs/week
Requirements:
- 2+ years case management (not utilization)
- Active MI RN license
- BSN required
- Discharge planning experience
- BLS (AHA)
Preferred:
- Experience in Mother Baby unit or general case management
- Familiarity with Care Manager Epic
