Core Medical-Perm is seeking a LPN / LVN Home Health for a job in Aurora, Colorado.

Job Description Requirements

Home Health Case Manager - LPN

REQUIRED

  • Current State Issued Licensed Practical Nurse (LPN) license
  • Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certification are American Heart Association and Red Cross.
  • Personal transportation, valid state issued drivers license, good driving record and auto insurance as required by law.
  • High School Diploma or GED

PREFERRED

  • 3+ yrs as LPN in a clinic or long term care setting with geriatrics experience
  • Knowledge of medications used in the treatment of geriatrics
