| CLIENT: | Creative Health Solutions - Astoria Health and Rehab PT 701 Overlook Drive Winter Haven Fl |
| SETTING: | SNF, IP, and OP mixed settings |
| START DATE: | July |
| GUARANTEE: | 35 hours |
| ASSIGNMENT LENGTH: | 13 Weeks |
| REQUESTED EXPERIENCE: | Prefer candidates with at least 1 yr. in SNF or Clinical rotation/externship in the SNF. MUST be strong in SNF caseload and able to work independently Currently using a traveler but they are not working out because of lack of experience |
| STAFF: | Total number of team members 15 and 1 Rehab Tech |
| EMR: | NetHealth / RehabOptima |
| CASELOAD / PATIENT SCHEDULING: | 90% Productivity but client understands caseload may vary and says average is 88% 2 hour orientation and expectation is to hit the ground running and meet productivity |

| LOCATION HIGHLIGHTS: | Winter Haven is a city in central Florida, known for its canal-linked Chain of Lakes.
