Rad Tech needed asapBERKSHIRE HEALTH SYSTEM WILL NOT CONSIDER ANY TRAVELER WHO RESIDES WITHIN 50 MILES OF FACILITY. To avoid delays in processing, please include perm address at profile upload. COVID VACCINATION (dose date on or after 10/3/23) NOW REQUIRED FOR THIS HEALTH SYSTEM. Exemption forms are available in Trio/Downloadable Forms (Both forms REQUIRE Provider/Religious Leader Signatures), or please ask your delivery team for documents. IF APPROVED BY HEALTHSYSTEM TRAVELER WILL WEAR A MASK AT ALL TIMES WHILE ON SITE.
