HIGHLIGHTS:
- Contract Length: 13 weeks
- Guaranteed Hours per Week: 38 hours flex up 40
- Shift Requirements: 8 variable days (Monday- Friday)
- Float Requirements: occasional travel to central/ Northern regions
- On Call/Call-Back: N/A
- Weekends: PTs rotate one Saturday every 6-8 weeks (will be given a a day off during the week on the weeks they work Saturday)

- Required Certifications: AHA BLS, Licensure required for the State of Wisconsin, Valid Drivers License (with proof of car insurance)
- Required Education: Bachelors Degree in Physical Therapy
- Minimum Experience: Home Health experience, training working with the adult/geriatric population

ADDITIONAL DETAILS:
- Must have NPI number (can apply upon offer)
- Contract is eligible for 36 hours of unbillable low census hours
- GSA hours are not guaranteed the week of Orientation
- Traveler is guaranteed set hours per week, except during holidays where unit is closed
- Contract is eligible for 36 unbillable low census hours
- Travelers can be put on call for low census hours
- The on-call hours that travelers are placed on will count toward the 36 low census unbillable hours
- If the traveler is called back to the facility, they will be able to receive call back pay
