  • Experience with UR tools (InterQual/MCG/CarePort)

Shift Notes

5 - 8.5hr Shifts with 1hr lunch

• Preferred/Additional:
  • PRI SCREEN Certifications
    • Available online via School of Albany
    • Duration: ~2 hours per certification

• Focus: Inpatient Discharge Planning (NOT Utilization Review)
• Primary Units: Medical-Surgical (MS) or Telemetry (Tele)
• Collaborates with: Social Worker (SW) , Physicians , and Interdisciplinary Team
• Expected Duties:
  • Rounding with physicians/team
  • Coordinating referrals (e.g., SNF, housing, hemodialysis, wound care/KCI)
  • Discharge planning to home, community, or facility
  • Managing complex social needs (e.g., homelessness support)
• Must be:
  • Independent
  • Critical thinker
  • Organized with strong time management
  • Team-oriented with a positive attitude

• Typical floor census: 33-40 patients
• Staffing:
  • Standard: 1 CM + 1 SW #134;#146; Caseload: 17-25 patients
  • Pilot Floors: 1 CM + 2 SWs #134;#146; Caseload: 12-17 patients
• Minimum expectation : Ability to handle at least 20 patients
• Common coverage: Internal Medicine and Family Medicine patients

• EMR : Allscripts
• Preferred Experience With :
  • InterQual
  • MCG (Milliman Care Guidelines)
  • CarePort
• Utilization Review : Handled by a separate department

• Hours : Monday-Friday, 8:00 AM-4:30 PM
  • (7.5-hour shift + 1-hour lunch break)
  • Management encourages taking a full lunch
• Weekends Holidays :
  • Rare, manager approval required
  • May involve covering multiple units

• Business Casual or Any Color Scrubs
  • Except: No Royal Blue or Maroon
  • Preferred: Neutral colors
• Lab coat provided

• Hospital Orientation : 2 days (includes EMR training)
• Unit Onboarding : 1-2 weeks with a preceptor (based on progress)

• No RT involvement in discharge planning
• Medication reconciliation pharmacy orders handled by physicians
