Signify Health, part of CVS Health, is hiring full-time clinicians to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial health plan members.
In this role, you will:
Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (majority of visits scheduled weekdays between 8 a.m. and 7 p.m.)
Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
Execute a strong visit completion rate, consistently pacing 110+ completed visits per month
Other role expectations and requirements:
Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Consistently engage with regular manager, team and clinical meetings
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Maintain all required supplies and associated diagnostic equipment related to DPS offerings
Maintain active state licenses in coverage areas
Maintain board certification (as applicable)
Maintain multi-state licenses (preferred)
Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
Be flexible to travel locally and within licensed states. May require hotel stay, car rental, and/or flight up to 20% annually
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required
Accommodate field observations and/or ridealong experiences, as requested
During our In-Home Health Evaluations (and other associated follow-up visits), youll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.
The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the persons health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter peoples current treatment regimen.

The typical pay range for this role is:
$78,331.00 - $168,714.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
