About UVMHN Home Health & Hospice

UVM Health Network Home Health & Hospice is a nonprofit home health and hospice provider. We are a mission driven, community-based organization whose focus is to deliver high quality care for all those in need, regardless of ability to pay.

We serve Chittenden and Grand Isle Counties.

For more than 100 years, we have provided high-quality, compassionate care wherever our community members call home. We support individuals and families at every age and stage of life, from pregnancy and early childhood care to adults with acute and chronic illnesses and those at the end of life.
