Our "See You In Seven" initiative is designed to enhance patient education and follow-up care for those with heart stents. Research indicates that patients who consult with a nurse practitioner within seven days of discharge experience improved outcomes. This program emphasizes medication education, including DAPT and nitroglycerin, activity guidelines, cardiac rehab enrollment, smoking cessation, and comprehensive treatment planning post-stent for coronary artery disease.

Once a patient is scheduled for a percutaneous coronary intervention (PCI), we arrange a follow-up clinic visit with a nurse practitioner. Our goal is to ensure every patient leaves with a "See You In Seven" card, detailing their next appointment and medication instructions, to support their recovery journey.
