Since our inception in 1978 with a single skilled nursing facility, we have expanded to 17 skilled nursing locations, 13 assisted living facilities, and two independent living communities across Western and Central New York, Vermont, and Rhode Island. Many Elderwood locations offer comprehensive rehabilitation and specialty subacute programs, including cardiac rehabilitation, respiratory care, pulmonary rehabilitation, memory care, and physical therapy.

Our skilled nursing facilities consistently meet the rigorous standards required to earn the prestigious Quality Award from the American Health Care Association.
