Our unique Passport approach connects residents with the appropriate clinical and rehabilitation care during their stay, supporting their recovery and treatment goals. This continuum of care extends beyond our facility, ensuring residents achieve their health objectives upon returning home. Key components of this approach include:

Passport to Home: A post-discharge transitional care program aimed at bridging the gap between facility care and home care.

Comprehensive Support: Services such as nurse practitioner visits within 24-48 hours post-discharge, telehealth consultations, remote patient monitoring, chronic care management, educational resources, and at-home therapy support.
