  • Passport to Home: A post-discharge transitional care program aimed at bridging the gap between facility care and home care.
  • Comprehensive Support: Services such as nurse practitioner visits within 24-48 hours post-discharge, telehealth consultations, remote patient monitoring, chronic care management, educational resources, and at-home therapy support.

Our center is led by Administrator Charlene Powers, a dedicated professional with a bachelor's degree in healthcare administration. With a career spanning since 1982 and administrative roles since 2011, Charlene exemplifies our commitment to excellence.
