Nationwide Therapy Group is seeking a travel nurse RN Case Management for a travel nursing job in Fort Defiance, Arizona.
Job Description Requirements

Registered Nurse (RN) - Case Manager / Care Coordinator
Facility: Tsehootsooi Medical Center
Department: Primary Care Clinic
Shift: Monday - Friday, Day Shift
Hours: 8:00 AM - 5:00 PM OR 7:30 AM - 4:30 PM (1-hour lunch)
Contract Length: 13 weeks (91 days)
Guaranteed Hours: 36 hours/week
Start Days: Orientation on Mondays only
No weekends or holidays
Billing String: Medefis
Position Details
- Title: Care Coordinator (RN Case Manager)
- Setting: Outpatient - Primary Care Clinic
- Schedule: Monday-Friday (No weekends or holidays)
- Lunch Break: 1 hour
- Call-Off Policy: Max 2 unapproved call-offs per contract
Start: Must meet compliance deadline to start on Monday orientation
Requirements
- Experience: Minimum 3 years of Case Management experience (REQUIRED)
- Licensure: Active RN license (state or compact as applicable)
- Certifications: BLS recommended
Travel Experience: Preferred but not required
Floating Expectations
May occasionally float to other departments based on qualifications
- Could include patient load or task-based duties (e.g., answering phones/call lights)
- Must be competent in floated settings
Housing Onboarding
- Housing Info: Available under the Facility Group Document section in Medefis
Orientation: Held Mondays only
- If compliance is not completed by the deadline, start will be delayed to the next orientation
Additional Notes
- Low acuity care setting with collaborative environment
- Work closely with providers and support nursing staff
- No holiday, weekend, or on-call requirements
