Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, youll be the face of our care-helping families say yes to compassionate end-of-life support when it matters most.

A minimum of one year of hospice experience is required for this role.

The schedule for this position is Monday-Friday 10am -7pm.

What Youll Do as a Hospice RN Admissions Nurse:

. Report directly to the Admissions Manager and serve as a key ambassador for our hospice services
. Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
. Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
. Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
. Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
. Use the nursing process to assess patient needs, support care planning, and document clinical findings
. Build trust and provide emotional reassurance to patients and families as they make this important decision
. Accurately complete required documentation and ensure compliance with state and federal regulations
. Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff

Qualifications - What Youll Bring:

. 1+ year of hospice nursing experience

. Active RN license in the state of employment
. A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
. Strong assessment, communication, and interpersonal skills
. Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
. Valid drivers license, auto insurance, and reliable transportation
. Current CPR certification

Preferred Experience (Not Required):

. 2+ years of hospice, home health, healthcare admissions, or medical sales
. Bachelor's Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
. Background in case management, care coordination, or healthcare outreach
. Experience with electronic medical records and compliance documentation
