What You’ll Need to Qualify

• Current BLS; CCM Certification
• Willingness to float throughout the hospital and/or multiple locations
• EPIC experience preferred
• Cannot hold a concurrent position at a Trinity Health facility
• Ability to complete orientation and training (flexible options available)

Essential Functions

• Demonstrates Trinity Health’s mission, vision, and values in leadership, behaviors, and decisions.
• Assesses, plans, evaluates, and coordinates management of patient care including discharge planning and utilization needs.
• Supports length of stay goals and Case Management initiatives by identifying delays and capacity issues.
• Coordinates with the interprofessional team to meet patient care goals.
• Partners with Patient Experience champions to align efforts with satisfaction outcomes.
• Identifies discharge needs and appropriate level of post-hospital care, coordinating continuity across settings.
• Ensures clinical information is provided to outside agencies as needed.
• Communicates effectively and professionally with patient care teams.
• Identifies and removes barriers to efficient patient management; promotes process improvements.
• Pursues professional development and maintains licensure.
• Advocates for patient rights while balancing resources and stewardship.
• Refers quality or utilization issues to appropriate channels.
• Facilitates patient and family education.
• Educates leadership and colleagues on case management practices and trends.
• Provides consultation on patient options, finances, psychosocial, risk, and ethical issues.
• Documents assessments, interventions, and outcomes per standards and policies.
• Uses computer systems effectively.
• Maintains knowledge of federal, state, and local regulations, Trinity Health policies, and compliance requirements.

Minimum Qualifications

• Valid nursing license as required by the state of practice
• Current BLS certification
• CCM certification (preferred)
• Prior Case Management or Care Coordination experience preferred
