Position: RN Case Manager
Shift: 5 days/week, Monday-Friday, 8-hour shifts (8:00 AM - 4:30 PM), 40 hours per week
Start Date: ASAP
Contract: 13 weeks (Temp-to-Hire option preferred)
License: MA license preferred; candidates with CA or NJ licenses not accepted due to delays. Will consider candidates willing to obtain MA license.
Certifications: BLS required
Education: BSN preferred but not required

Job Summary:
The RN Case Manager is responsible for discharge planning and case management in an acute care setting. This position does not include utilization review responsibilities but does require experience with discharge planning and familiarity with Medicare, Medicaid, managed care, and third-party payor requirements.

Key Responsibilities:

• Coordinate discharge planning and ensure continuity of care post-discharge.
• Verify medical necessity of hospitalization and treatment per hospital and third-party payor criteria.
• Collaborate with physicians and the interdisciplinary healthcare team to manage patient care efficiently and cost-effectively.
• Act as liaison between medical staff, departments, patients, families, and community resources.
• Arrange services for patients at discharge to meet planned care outcomes.
• Advocate for patients and families requiring post-hospital services.

Required Experience:

• Prior experience in a Med-Surg setting.
• Experience in discharge planning and/or case management.
• Familiarity with Medicare, Medicaid, managed care, and third-party payor review requirements.
