License Required: WA RN license or compact
Experience Required: Minimum of 2 years in home health, with recent experience within the last 10 years. Familiarity with Home Care Home Base is strongly preferred.
Responsibilities:
. Provide comprehensive nursing care to patients in a home health setting.
. Collaborate with our clients team to develop and implement patient care plans.
. Utilize the Home Care Home Base EMR system for documentation.
. Conduct patient visits and manage a caseload using a point system.
