GHR is hiring a full time Home Health Physical Therapist in Bordentown, NJSchedule: Monday - Friday, 8:00 AM - 4:30 PMLocation: Regular home visits in Mercer County North Burlington CountyNo on-call duties or weekend work requiredTravel rate: $2824 gross per weekLocal: $66-69/hrThis role involves providing therapy services in participants homes, creating personalized treatment plans, and collaborating with a dedicated care team.Key Responsibilities: . Conduct patient evaluations and develop individualized treatment plans. . Provide therapy to improve mobility, function, and overall quality of life. . Educate patients and caregivers on exercises and home-based care. . Work closely with physicians, nurses, and case managers. . Maintain accurate and timely documentation.Qualifications: . Graduate of an accredited Physical Therapy program. . Licensed Physical Therapist in NJ. . AHA BLS Certification required. . Minimum 2 years of experience working with adults. . Experience in home health and senior care is essential. . Strong communication, documentation, and problem-solving skills. . Reliable transportation required.Preferred Qualifications: . Experience in pelvic floor therapy. . Training in Herman Wallace Level 1A or equivalent.Physical Demands Work Environment: . Frequent standing, walking, and driving. .
