• Provide skilled nursing care following the active Plan of Care prescribed by physicians or healthcare professionals

• Educate patients and families on health conditions, disease management, prevention, and self-care techniques
• Collaborate with interdisciplinary teams to ensure continuity and quality of care
• Support patients with individualized 1:1 caseloads, providing personalized attention and care
