• The Nurse Navigator acts as a consultant, facilitator and educator to the Imaging service line patient population.
• Responsibilities include assisting patients through the Imaging care delivery system through patient education, coordination of care, patient advocacy and communication with the multi-disciplinary team as well as the patient and family as appropriate.
• The navigator will maintain communication with referring providers and be comfortable with discussing best practice guidelines and standard of care.
• Navigation responsibilities include maintaining an ongoing database on patients and be comfortable generating reports related to program metrics.

What qualifications you will need:

• Active NH Registered Nurse license or compact Registered Nurse license
• Associate Degree in Nursing
• 3 - 7 years of relevant work experience
• Basic Cardiac Life Support must be obtained within 30 days of employment start date
