Planet Healthcare is seeking a travel nurse RN Acute Care Case Management for a travel nursing job in Bronx, New York.

Job Description Requirements

Specialty: Acute Care Case Management

Shift: 8 hours, days
Employment Type: Travel

-M-F 8-4:30 (7.5 Hr shifts with a 1 hr lunch)-Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker-Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning-Must have 1 year of inpatient case management experience. Prefer prior critical care experience-Occasional weekends/holidays may be needed-BLS, PRI requiredNY RN license required
