Case Management Discharge Planning: Admission criteria, care coordination, plan of care, DME ordering, prior authorizations, insurance/benefits eligibility
Knowledge of Services: Adult/child protective services, hospice/advance directives, Medicaid, Medicare, VCP, community resources
Clinical Knowledge: CHF, COPD, Stroke, Dementia, Diabetes, HIV/AIDS, Neurological disorders, Psychiatric conditions
Care Coordination: Internal/external team collaboration, referrals, patient/family education, documentation in Epic
Work Settings: ED, ICU, Med-Surg, LTC, Hospice, Home Health
