ADDITIONAL DETAILS:
- COVID-19 Vaccine: Required - Medical/Religious Exemptions Only
- Flu Vaccine: Unknown
- All RTO must be included on the initial submission. RTO requested after the fact will not be considered
- Compliance documents are due 5 business days prior to the travelers scheduled start by EOD eastern time
- Candidate must go into the lab within 24 hours after signing
- Pass/Fail testing required:
- Cardiac Dysrhythmia Test and Moderate Sedation Test will be given on Monday
- Travelers have ONE chance to pass
- Cardiac Dysrhythmia Test required for all units
- Moderate Sedation Test required for OR, ICU, and Endo
- Attendance policy: If more than two shifts per 13-week contract are missed without reasonable explanation subject to SRHS discretion, contract will be reviewed for cancellation
