About JourneyCare Hospice

JourneyCare helps people facing serious illness live life to the fullest, with dignity and without pain, on their own terms. We care for the whole person body, mind and spirit not just the disease, and work together with the patient, family and other care partners to create a customized care plan that supports and respects their goals, preferences and choices.

Care is provided wherever the patient calls home a private residence, senior living community, a long-term care facility or hospital and a team of trained nurses is available 24 hours a day, seven days a week. If pain and symptoms become too difficult to manage at home, around-the-clock care can be provided in one of our three inpatient hospice care centers.

JourneyCare has a deep understanding of our community. We are committed to education, for both community members and healthcare professionals. JourneyCare also partners with area colleges and universities to offer accredited fellowships, residency programs and student rotations.
