Today, American Health Communities (AHC) operates 29 senior living and rehabilitation communities across Tennessee and Alabama. Being part of this network of communities means AHC staff can draw upon the resources and expertise of our corporate clinical services group, who oversees and supports all providers.

Local AHC providers also work closely with other American Health Partners affiliatesTennessee Quality Care Home Health and Hospice, Rehab America, and Unity Psychiatric Care to provide a continuum of post-acute care. Through collaborative partnerships we expand residents access to high-quality health services and ease transitions to different types of care, delivered in other settings.

In addition, AHC long-term residents may be eligible to join our affiliated health plan, American Health Advantage of Tennessee. This specialized Medicare Advantage plan offers coordinated care and special benefits for Medicare beneficiaries living in nursing homes.
