Most importantly, our employees are appreciated for the real differences they make in both the lives of their clients and their clients' families.</p><p>Job Summary:</p><p>Under the clinical supervision of a Physical Therapist, the Physical Therapist Assistant administers Physical Therapy treatments to patients. Provides assistance to the Physical Therapist by supplying objective measures to be utilized by the Physical Therapist in evaluation of patient's level of function and determination of treatment strategies.</p><p>May work in an Inpatient, Outpatient, Home Care, Skilled Nursing, or Assisted Living environment<br><br>Responsibilities: </p><ul><li>Assists the Physical Therapist with aspects of patient assessment by performing delegated duties such as testing for joint range of motion, muscle strength, and mobility status, identifying rehab precautions.</li><li>Recognizes physiological and age related changes in the patient. Consults with the Physical Therapist and other Health Care Team members.</li><li>Under the clinical supervision of the Physical Therapist and as delegated by the Physical Therapist, perform therapeutic techniques that are appropriate to patient's needs and medical status. Implement treatment plan as provided by the Physical Therapist and deliver patient care by utilizing appropriate interventions, such as heat, cold, light, water, sound, electricity, mobilization and therapeutic exercise.</li><li>Communicate patient progress to the Physical Therapist.</li><li>Provides patient and caregiver education regarding treatment, precautions, and equipment use as delegated by the Physical Therapist.</li><li>Completes all documentation including but not limited to: evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary. </li><li>Assist the Physical Therapist with patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care. Help to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and other community resources as related to care transition planning.</li></ul></div><p><b>Qualifications</b> </p> <p>Education Training: </p><p>Associates Degree from an accredited Physical Therapy Assistant program</p><p>License/Certification:</p><p>Current State of CT Physical Therapy Assistant license required</p><p>Basic Life Support (BLS)</p><p>(BLS upon hire or within 30 days of hire: varies by facility)</p><p>For Home Care Only: Valid driver's license and reliable transportation required<br>Experience: </p><p> Previous experience in acute care or outpatient setting preferred<br>Skills:<br> . Knowledge of principles and practices of Physical Therapy.<br> . Excellent communication and organizational skills.<br> . Excellent interpersonal skills; ability to interact with patients, families and co-workers.<br> .
